AN ACT

 

amending Title 15, chapter 1, article 1, Arizona Revised Statutes, by adding section 15-118; relating to vision screenings.

Be it enacted by the Legislature of the State of Arizona:

Section 1.  Title 15, chapter 1, article 1, Arizona Revised Statutes, is amended by adding section 15-118, to read:

START_STATUTE15-118.  Annual vision screening; parental notification; database

A.  Each school district and charter school shall annually provide a vision screening to each pupil in a kindergarten program and each of grades one, two, three, four, five and nine.  the screenings shall use a Snellen chart, or an equivalent screening device such as an automated vision screening device, and shall be provided at no cost to the school district or charter school.

B.  The superintendent of the school district and the principal of the charter school shall give written notice to the parent or guardian of a pupil who:

1.  is found to have a possible defect of vision or disease of the eye, with a brief statement describing the possible defect or disease.

2.  Did not receive a vision screening pursuant to this section, with a brief statement explaining why the pupil did not receive the vision screening.

C.  within ten days after a vision screening pursuant to this section, The superintendent of the school district and the principal of the charter school shall provide to the department of education statistical information by grade level of the number of pupils screened and the number of pupils referred for follow-up examinations.  The department shall maintain a central database for this information.
