       Senator Osgood moved the following:
       
    1         Senate Amendment to Amendment (333316) (with title
    2  amendment)
    3  
    4         Delete lines 230 - 249
    5  and insert:
    6  county.
    7         (a) The card must contain:
    8         1.(a) Voter’s registration number.
    9         2.(b) Date of registration.
   10         3.(c) Full name.
   11         4.(d) Party affiliation.
   12         5.(e) Date of birth.
   13         6.(f) Address of legal residence.
   14         7.(g) Precinct number.
   15         8.(h) Polling place address and a link to the supervisor’s
   16  website to provide the most current polling place locations.
   17         9.(i) Name of supervisor and contact information of
   18  supervisor.
   19         10.(j) Other information deemed necessary by the
   20  supervisor.
   21         (b) A voter information card may not be furnished by the
   22  supervisor to a voter unless the supervisor or the division has
   23  determined that the voter is eligible to vote.
