Section 514C.4, subsection 2, Code 2015, is 1 2 amended to read as follows: 1 3 2. a. As used in this section, "minimum mammography 1 4 examination coverage" means benefits which are better than or 1 5 equal to the following minimum requirements: 1 6a.(1) One baseline mammogram for any woman who is 1 7 thirty=five through thirty=nine years of age, or more frequent 1 8 mammograms if recommended by the woman's physician. 1 9b.(2) A mammogram every two years for any woman who is 1 10 forty through forty=nine years of age, or more frequently if 1 11 recommended by the woman's physician. 1 12c.(3) A mammogram every year for any woman who is fifty 1 13 years of age or older, or more frequently if recommended by the 1 14 woman's physician. 1 15 b. For purposes of this section, "minimum mammography 1 16 examination coverage" shall include coverage of 1 17 three=dimensional mammography. 1 18 Sec. 2. APPLICABILITY. This Act applies to third=party 1 19 payment provider policies or contracts issued for delivery, 1 20 continued, or renewed in this state on or after January 1, 1 21 2016. 1 22 EXPLANATION 1 23 The inclusion of this explanation does not constitute agreement with 1 24 the explanation's substance by the members of the general assembly. 1 25 This bill provides that mandated health care coverage 1 26 benefits for mammography must include coverage of 1 27 three=dimensional mammography. The bill is applicable to 1 28 third=party payment provider policies or contracts issued for 1 29 delivery, continued, or renewed in this state on or after 1 30 January 1, 2016.
