| 1 | AMENDMENT TO SENATE BILL 3137 | ||||||
| 2 | AMENDMENT NO. ______. Amend Senate Bill 3137 on page 1, by | ||||||
| 3 | replacing lines 11 and 12 with the following: | ||||||
| 4 | "representative, if known, of the patient as specified in this | ||||||
| 5 | Section. The facility shall attempt to provide verbal notice | ||||||
| 6 | to the personal representative, if known, of the patient"; and
| ||||||
| 7 | on page 2, by replacing lines 8 and 9 with the following: | ||||||
| 8 | "representative, if known, of the recipient of services as | ||||||
| 9 | specified in this Section. The facility shall attempt to | ||||||
| 10 | provide verbal notice to the personal representative, if"; and
| ||||||
