It is an editor's brief statement of the essential features of the measure as introduced. Requires coordinated care organization to contract with area agencies to coordinate transition to long term care from health care facility of members who are dually eligible for Medicare and Medicaid. Declares emergency, effective on passage. A BILL FOR AN ACT Relating to area agencies; and declaring an emergency. Be It Enacted by the People of the State of Oregon: SECTION 1. { + Section 2 of this 2013 Act is added to and made a part of ORS chapter 414. + } SECTION 2. { + (1) As used in this section: (a) 'Area agency' has the meaning given that term in ORS 410.040. (b) 'Health care facility' has the meaning given that term in ORS 442.015. (2) A certified coordinated care organization must contract with the area agencies located in the geographic region served by the organization to provide transitional care coordination for members of the organization who are ready to be discharged from a health care facility and who are dually eligible for Medicare and Medicaid. The organization must contract with the area agencies to ensure that upon discharge there is a transition plan in place for long term care and in-home support as necessary for the member to live at home or in the community. + } SECTION 3. { + This 2013 Act being necessary for the immediate preservation of the public peace, health and safety, an emergency is declared to exist, and this 2013 Act takes effect on its passage. + } ----------
