Amend HB 625 (house committee report) as follows:

(1)  On page 1, line 12, between "therapist" and the underlined period, insert "if that visit did not require a referral from a physician or provider".

(2)  On page 1, line 19, between "therapist" and the underlined period, insert "if that visit did not require a referral from a physician or health care provider".
