An Act

 

amending title 36, chapter 5.1, article 1, Arizona Revised Statutes, by adding section 36-576; relating to developmental disabilities.

Be it enacted by the Legislature of the State of Arizona:

Section 1. Title 36, chapter 5.1, article 1, Arizona Revised Statutes, is amended by adding section 36-576, to read:

START_STATUTE36-576. Division duties; verification system for service and treatment plans; billing statements

The division shall:

1. develop and implement a verification system for person-centered service plans and behavioral treatment plans to ensure information accuracy and consistency.

2. Implement an independent review process for monthly residential billing statements.

3. Require residential billing statements to include at least the following information:

(a) A brief, clear, nonmisleading, plain language description of the service or services rendered to accompany each charge.

(b) The name of the service provider associated with each charge.

(c) A clear indication if there is a service provider change.END_STATUTE
