HOUSE CONCURRENT

RESOLUTION

 

 

DECLARING THE SECOND WEEK IN OCTOBER AS CASE MANAGEMENT WEEK.

 

 

     WHEREAS, the World Health Organization has defined "health" as "a state of complete physical, mental and social well-being and not merely the absence of disease or infirmity"; and

 

     WHEREAS, the issues of health and access to healthcare have been the topic of much current debate both on a national and state level; and

 

     WHEREAS, the modern healthcare environment is a maze of complex medical, social, and fiscal considerations; and

 

     WHEREAS, case management is a collaborative process that includes assessment, planning, facilitation, and advocacy for options and services to meet an individual's health needs; and

 

     WHEREAS, case managers provide critical services on a daily basis to help patients and their caregivers navigate the modern healthcare landscape to better achieve their healthcare goals; and

 

     WHEREAS, while pursuing their primary goal of effective patient care, case managers advocate for patient needs, develop individualized plans of care, coordinate services between providers, and facilitate the scheduling of services; and

 

     WHEREAS, in addition to helping patients achieve wellness and autonomy, case managers are also adept at ensuring that available resources are being used in a timely and cost-effective manner to obtain optimum value for both the client and the reimbursement source; and

 

     WHEREAS, case management has grown substantially as a profession over the past decade, with over 40,000 practitioners holding a Certified Case Manager credential; and

 

     WHEREAS, case managers can be found in multiple settings including government agencies, health insurance companies, hospitals, and private managed care companies; now, therefore,

 

     BE IT RESOLVED by the House of Representatives of the Twenty-fifth Legislature of the State of Hawaii, Regular Session of 2010, the Senate concurring, that the second week in October, beginning with Sunday, be designated "Case Management Week," to recognize the case management profession as a unique and effective way to mobilize resources to promote cost-effective, quality outcomes for a wide range of populations with varied healthcare needs; and

 

     BE IT FURTHER RESOLVED that this celebratory week should include activities that recognize the contributions and commitments case managers make to their clients, in addition to educating the public, caregivers, healthcare industry, and elected officials about the significant work performed by these dedicated healthcare professionals; and

 

     BE IT FURTHER RESOLVED that certified copies of this Concurrent Resolution be transmitted to the Governor, Director of Health, Director of Human Services, and President of Hawaii Chapter of the Case Management Society of America.
