health

shall
submit
a
request
to
the
centers
for

Medicaid
services
of
the
United
States
department

of
health
and
human
services
for
a
waiver
pursuant
to
the

Medicaid
reentry
section
1115
demonstration
opportunity,

created
pursuant
to
section
5032
of
the
federal
Substance
7
Use-Disorder
Prevention
that
Promotes
Opioid
Recovery
and

Treatment
for
Patients
and
Communities
Act,
or
SUPPORT
for
9
Patients
and
Communities
Act,
Pub.
L.
No.
115-271,
to
provide
10
coverage
and
services
to
facilitate
continuity
of
care
and
11
improve
care
transitions
to
the
community
for
individuals
12
who
are
soon
to
be
former
inmates
of
a
public
institution,

as
inmate
of
a
public
institution
is
defined
in
42
C.F.R.

§435.1010,
and
who
are
otherwise
eligible
for
Medicaid.
