AMENDMENT TO HOUSE BILL 491
AMENDMENT NO. ______. Amend House Bill 491 by replacing
everything after the enacting clause with the following:
"Section 5. The School Code is amended by adding Section
22-80 as follows:
(105 ILCS 5/22-80 new)
Sec. 22-80. Student athletes; injury incident report.
(a) In this Section, "official" means a patched official as
defined by guidelines of the Illinois Elementary School
Association or the Illinois High School Association for
certification.
(b) All officials certified by the Illinois Elementary
School Association or the Illinois High School Association
shall fill out an injury incident report following a game if an
athlete is unable to return to the field of play due to an
injury received during the game. The report shall include

whether the injury occurred to the upper or lower extremities
and a special notation for concussive or other head injuries.
(c) The Illinois Elementary School Association and the
Illinois High School Association shall maintain a database of
the reports required under this Section and file the reports,
along with the identification and contact information of the
officials that filed out the reports, with the Department of
Public Health at the end of each respective athletic season.
Section 99. Effective date. This Act takes effect July 1,
2013.".
