Summary

Create a new section of Subtitle 17A of KRS Chapter 304 to require the executive director of the Office of Insurance to develop uniform application forms for individual, small group, and large group health benefit plans on or before August 1, 2010 in cooperation with the Health Insurance Advisory Council, and considering recommendations from insurers offering health benefit plans in the state; require that the uniform application forms be in plain language, include common demographic information to the class of health benefit plans, and be made available in a printed and electronic format; require the executive director to adopt the forms by promulgation of an administrative regulation; and require all health benefit plans to use the appropriate uniform application form on and after January 1, 2011.
